Sonoma Valley Health Partners (Sonoma Valley Hospital, Sonoma Valley Community Health Center, Sonoma Valley Fire & Rescue, Schell-Vista Fire Protection District, Sonoma Valley Unified School District, Sonoma County Department of Public Health, and the Sonoma Valley Catalyst Fund) are providing the COVID-19 Vaccine in Sonoma Valley.
ELIGIBILITY FOR UPCOMING CLINICS (as of 2/16/2021)
Phase 1(a) – All Tiers (1, 2, 3) Sonoma County Healthcare workers with proof of workplace eligibility (must live or work in Sonoma County).
Phase 1(b) Sonoma County residents age 70 and older
Phase 1(b) Food & Agriculture PRODUCTION (Farmers, Skilled Agriculture, Plant Production) that live or work in Sonoma County (no grocery store workers, restaurant, hospitality, food and beverage sales/service at this time)
Phase 1(b) Childcare Essential Workers and Education Essential Workers (must live or work in Sonoma County).
These clinics are for the first dose only. The second dose will be scheduled, following the inoculation.
Due to limited supply, we will expand our reach as supply and County/State Guidelines allow.
CANCELATIONS: Appointments are subject to cancellation for those that who do not meet eligibility requirements.

Vaccinations are by appointment only. If you need assistance with online registration, please see below for resources prepared to assist with your registration.
If all appointments are filled by the time you finish the registration, please check for availability on other days through the other links provided. The registration system will stop scheduling appointments once all are taken. Unfortunately, we are unable to accommodate walk-up appointments.

IMPORTANT: You must also commit to your second dose appointment. Second dose appointments will be made on site following your first dose.
Vaccine Type (Pfizer or Moderna) is subject to change without notice. However, your first and second dose appointments will be for the same brand of vaccine (either both Pfizer or both Moderna).
For those 70 years and older, an ID or other document verifying
your age is required.
For Workers in Phase 1(a) (all tiers) and Phase 1(b) evidence of eligibility include a work ID badge or letter from your place of employment.
We are unable to accommodate those without an appointment and those that have not registered
