Sonoma Valley Hospital to Launch MyChart Patient Portal

Connectivity With UCSF Health And Affiliates to Improve

On Saturday, December 3, 2022, Sonoma Valley Hospital transitions to a new online patient portal called MyChart. The hospital’s new electronic health record will connect healthcare providers through one system and provide SVH patients 24/7 access to their SVH medical records. MyChart replaces the current portals “Follow My Health” and “Pay My Bill” and can be accessed either by desktop computer or from most mobile devices using the downloadable MyChart app.

“MyChart is the most widely used patient portal across the United States, providing the optimal connection for patients and providers,” said John Hennelly, SVH CEO. “It is designed to improve and grow communication between our patients’ healthcare teams and across various healthcare networks, including UCSF Health.”

Through MyChart, patients will be able to better manage their family’s healthcare. The user-friendly dashboards allow patients or their medical guardians to access lab test results and, and other medical information from the hospital including:

  • Lab results
  • Medical Imaging results (Mammo, CT Scan, X-Ray, Ultrasound, etc.)
  • Post-care instructions following inpatient discharge
  • Emergency Department treatments, results, and follow-up care
  • Payments, statements, and invoices
  • Language options available

Activation codes will be sent to those SVH patients who have given consent to be communicated by email or text, beginning December 3, 2022. After December 3, any patient will be able to register and create their own MyChart account. Those patients already using MyChart will be able to link the SVH MyChart iteration to theirs.

 “After we go-live, staff may spend a few more minutes with documentation as we become more proficient,” said Hennelly. “MyChart is a proven communication tool, providing a central location for your medical information, which is a major reason to convert. We are very excited for our community, patients, care providers, and for the hospital.”

Beginning December 3, patients with questions about MyChart can call the MyChart Patient Engagement Team at 1-833-395-2035 Monday – Friday: 7:00 am – 7:00 pm and Saturday – Sunday: 9:00 am – 5:00 pm. Interpretation services will be made available.
